In Abhängigkeit vom Dialyseverfahren und dem jeweiligen Krankheitsverlauf ist die
Beziehung des Fachpersonals zum chronisch kranken Patienten von besonderer Bedeutung:
Sie sind meist langjährig, intensiv, vertraut und manchmal auch „heimisch“ geprägt.
Die latente, existenziell bedrohliche Lebenssituation in Verbindung mit der Abhängigkeit
von einer dauerhaften apparativen Versorgung bestimmt die Beziehung zum Fachpersonal
in besonderer Weise: Frühe Beziehungsmuster und Erwartungen werden reaktiviert und
unbewusst auf das Fachpersonal übertragen. Dabei nehmen der Patient, der Arzt und
die Pflegekräfte in der hierarchisch strukturierten Beziehung unterschiedliche Rollen
ein, sie teilen nicht dieselbe Wirklichkeit. Das Wissen um die psychischen Prozesse
des Patienten und die Auswirkungen im Praxisalltag, sowie seine Reflexionsfähigkeit
über das eigene Erleben und Handeln haben einen wesentlichen Einfluss auf die Compliance
und auf den Behandlungserfolg der Patienten. Die vorliegende Untersuchung soll zum
Verständnis der Beziehungsdynamik zwischen Patient und Fachpersonal beitragen.
Depending on the dialysis procedure and course of the disease, the relationship of
the professional staff to the chronically ill patients is of special importance: It
is usually long-lasting, intense, intimate and sometimes home-based. The latent existentially
threatening situation in conjunction with the dependency on permanent care by a technical
apparatus determines the relationship with the professional staff in a unique way:
Early relationship patterns and expectations are reactivated and unintentionally transferred
to the specialist staff. Patient, doctor and nurses take different roles in this hierarchically
structured relationship, they do not share the same reality. The knowledge about the
mental processes of the patient and their impact on daily practice, as well as the
ability to reflect on one's own experience and action have a major effect on the patient's
compliance and treatment success. The present investigation shall contribute to the
understanding of the relationship dynamics between patient and medical staff.
